Introducción: Para disminuir la aparición de hiponatremias en los últimos años se aumentaron las concentraciones de sodio en las soluciones de mantenimiento, llegando a recomendarse las isotónicas, con mejoras de laboratorio pero con dudoso impacto clínico. En el Hospital Garrahan se utiliza una solución estándar hipotónica con cloruro de sodio 0,45%. Antes de reemplazar la solución según recomendaciones internacionales se decidió establecer la prevalencia de hiponatremia en pacientes internados, y su asociación con la solución estándar de hidratación. Población y métodos: Pacientes de 1 mes a 18 años, internados en el Hospital Garrahan. Estudio prospectivo y observacional. Se registró si el paciente recibía hidratación parenteral y la concentración de sodio. Se consideró hiponatremia significativa la presencia de sodio sérico menor a 130 mEq/L. y/o la presencia de síntomas compatibles con hiponatremia. Resultados: En 3003 internaciones la prevalencia global de hiponatremias diagnosticadas fue 4.4%, y asciende a 6.3% si se consideran solo los pacientes que fueron estudiados con ionograma (se le extrajo ionograma al 70,6% de los pacientes internados). La prevalencia de hiponatremias significativas fue de 1.5% (n=44) de los internados, y las hiponatremias significativas en internados que recibían la solución hipotónica estándar de mantenimiento fue de 0.3% (n=9). Conclusiones: En una población donde se utiliza una solución estándar con cloruro de sodio 0,45% -pero se modifica individualmente para las necesidades de cada paciente- la prevalencia de hiponatremias totales y significativas fue similar e incluso inferior a la publicada en otras series. Introduction: To reduce the appearance of hyponatremia, in recent years, sodium concentrations were increased in maintenance solutions, and isotonic solutions were recommended, leading to improvements in laboratory studies, but with a doubtful clinical impact. A standard hypotonic solution with 0.45% sodium chloride is used at Garrahan Hospital. Before replacing the solution according to international recommendations, it was decided to determine the prevalence of hyponatremia in inpatients and its association with the standard hydration solution. Population and methods: Patients from 1 month to 18 years old, hospitalized at Garrahan Hospital. Prospective and observational study. Parenteral hydration of the patient and the sodium concentration were recorded. Significant hyponatremia was defined as serum sodium less than 130 mEq/L, and/or the presence of symptoms of hyponatremia. Results: In 3003 hospitalizations, the overall prevalence of diagnosed hyponatremia was 4.4%, increasing to 6.3% if only patients in whom a ionogram was performed were included (a ionogram was performed in 70.6% of the inpatients). Of all inpatients, 1.5% (n=44) had significant hyponatremia, and 0.3% (n=9) of the patients receiving the standard maintenance hypotonic solution had significant hyponatremia. Conclusions: In a population in whom a standard solution with 0.45% sodium chloride is used - but which is individually modified according to the needs of each patient - the prevalence of total and significant hyponatremia was similar and even lower than that reported in other series (AU)

Background and objectives: Guillain-Barré Syndrome is one of the most common causes of acute polyneuropathy in adults. Recently, the occurrence of Guillain-Barré Syndrome after major and minor surgical operations has been increasingly debated. In Guillain-Barré syndrome, syndrome of inappropriate antidiuretic hormone secretion and dysautonomy are generally observed after maximal motor deficit. Case report: A 44-year-old male patient underwent a laparoscopic cholecystectomy for acute cholecystitis. After the development of a severe headache, nausea, diplopia, and attacks of hypertension in the early postoperative period, a computer tomography of the brain was normal. Laboratory tests revealed hyponatremia linked to syndrome of inappropriate antidiuretic hormone secretion, the patient's fluids were restricted, and furosemide and 3% NaCl treatment was initiated. On the second day postoperative, the patient developed numbness moving upward from the hands and feet, loss of strength, difficulty swallowing and respiratory distress. Guillain-Barré syndrome was suspected, and the patient was moved to intensive care. Cerebrospinal fluid examination showed 320 mg/dL protein, and acute motor-sensorial axonal neuropathy was identified by electromyelography. Guillain-Barré syndrome was diagnosed, and intravenous immune globulin treatment (0.4 g/kg/day, 5 days) was initiated. After 10 days in the intensive care unit, at which the respiratory, hemodynamic, neurologic and laboratory results returned to normal, the patient was transferred to the neurology service. Conclusions: Our case report indicates that although syndrome of inappropriate antidiuretic hormone secretion and autonomic dysfunction are rarely the initial characteristics of Guillain-Barré syndrome, the possibility of postoperative syndrome of inappropriate antidiuretic hormone secretion should be kept in mind. Justificativa e objetivos: a síndrome de Guillain-Barré é uma das causas mais comuns de polineuropatia aguda em adultos. Recentemente, a ocorrência após grandes e pequenas intervenções cirúrgicas tem sido cada vez mais debatida. Na Guillain-Barré, a síndrome da secreção inapropriada do hormônio antidiurético e a disautonomia são geralmente observadas após déficit motor máximo. Relato de caso: paciente do sexo masculino, 44 anos, submetido a colecistectomia videolaparoscópica para colecistite aguda. Após desenvolver uma forte dor de cabeça, náusea, diplopia e ataques de hipertensão no período pós-operatório imediato, uma tomografia computadorizada do cérebro revelou-se normal. Os exames laboratoriais revelaram hiponatremia associada à síndrome de secreção inadequada de hormônio antidiurético; os líquidos foram restritos e tratamento com furosemida e NaCl a 3% foi iniciado. No segundo dia pós-operatório, o paciente desenvolveu dormência que se propagava a partir das mãos e dos pés, perda de força, dificuldade para engolir e respirar. Suspeitou-se de síndrome de Guillain-Barré e o paciente foi transferido para a unidade de tratamento intensivo. Exame do líquido cefalorraquidiano revelou 320 mg/dL de proteína e neuropatia axonal sensório-motora aguda foi identificada por eletromiografia. Justificación y objetivos: el síndrome de Guillain-Barré es una de las causas más comunes de polineuropatía aguda en adultos. Recientemente, la aparición del síndrome de Guillain-Barré después de cirugías mayores o menores se ha convertido en objeto de debate cada vez mayor. En el síndrome de Guillain-Barré, generalmente se observan síndrome de secreción inapropiada de hormona antidiurética y disautonomía después de un déficit motor máximo. Relato de caso: paciente de sexo masculino, 44 años, sometido a colecistectomía laparoscópica por colecistitis aguda. Después de comenzar con un fuerte dolor de cabeza, náuseas, diplopía y ataques de hipertensión en el período postoperatorio inmediato, una tomografía computadorizada del cerebro del paciente se reveló normal. Las pruebas de laboratorio revelaron hiponatremia asociada con el síndrome de secreción inadecuada de hormona antidiurética; los líquidos se restringieron y se inició tratamiento con furosemida y NaCl al 3%. En el segundo día del postoperatorio, el paciente presentó adormecimiento que se propagaba desde las manos y los pies, pérdida de fuerza, dificultad para tragar y para respirar. Se sospechó síndrome de Guillain-Barré y el paciente fue derivado a la unidad de cuidados intensivos. El examen del líquido cefalorraquídeo reveló 320 mg/dL de proteína y mediante electromiografía se identificó neuropatía axonal sensorial y motora aguda. ABSTRACT

Las disnatremias son el transtorno hidroelectrolítico prevalente en pacientes ambulatorios y hospitalizados. Su manejo inadecuado puede tener serias consecuencias, asociándose a un aumento en la morbimortalidad de los pacientes. El objetivo de este artículo es actualizar las bases fisiopatológicas de las disnatremias y revisar herramientas clínicas y de laboratorio que nos permitan realizar un enfrentamiento rápido y simple. Las disnatremias reflejan un transtorno del balance del agua, y el balance de agua tiene relación directa con la osmorregulación. Existen mecanismos para mantener el control de la osmolalidad plasmática, los cuales se gatillan con cambios de un 1-2 por ciento. A nivel hipotalámico existen osmorreceptores que censan cambios en la osmolalidad plasmática, regulando la secreción de Hormona Antidiurética (ADH), la que ejerce su acción a nivel renal, por lo cual el riñón es el principal regulador del balance hídrico. Cuando se está frente a una disnatremia, es fundamental evaluar cómo está funcionando este eje ADH-riñón. Dentro de las hiponatremias existen causas que son fáciles de identificar, sin embargo, diferenciar un síndrome de secreción inadecuada de ADH con un síndrome pierde sal cerebral suele ser más difícil. En el caso de las hipernatremias, sospechar una diabetes insípida y diferenciar su posible origen, central o nefrogénico, es fundamental para su manejo. En conclusión, el enfrentamiento de una disnatremia requiere conocer las bases fisiopatológicas de su desarrollo, para así poder realizar un diagnóstico certero y finalmente un tratamiento adecuado, evitando errores en su corrección que pueden poner en riesgo al paciente.


Dysnatremia is among the most common electrolyte disorders in clinical medicine and its improper management can have serious consequences associated with increased morbidity and mortality of patients. The aim of this study is to update the pathophysiology of dysnatremia and review some simple clinical and laboratory tools, easy to interpret, that allow us to make a quick and simple approach. Dysnatremia involves water balance disorders. Water balance is directly related to osmoregulation. There are mechanisms to maintain plasma osmolality control; which are triggered by 1-2 percent changes. Hypothalamic osmoreceptors detect changes in plasma osmolality, regulating the secretion of Antidiuretic Hormone (ADH), which travels to the kidneys resulting in more water being reabsorbed into the blood; therefore, the kidney is the main regulator of water balance. When a patient is suffering dysnatremia, it is important to assess how his ADH-renal axis is working. There are causes of this condition easy to identify, however, to differentiate a syndrome of inappropriate ADH secretion from cerebral salt-wasting syndrome is often more difficult. In the case of hypernatremia, to suspect insipidus diabetes and to differentiate its either central or nephrogenic origin is essential for its management. In conclusion, dysnatremia management requires pathophysiologic knowledge of its development in order to make an accurate diagnosis and appropriate treatment, avoiding errors that may endanger the health of our patients.

Analisar pacientes com traumatismo raquimedular (TRM) e os níveis séricos de sódio nos 14 primeiros dias pós-trauma, tratamento e prognóstico. Método: Trata-se de um estudo prospectivo, longitudinal e descritivo realizado no Serviço de Neurocirurgia do Hospital de Urgência de Sergipe (HUSE). Resultados: Foi prevalente o sexo masculino (80%). A idade média foi de 38,4. Quanto mais alto o nível da lesão, maior relação com o surgimento de hiponatremia. Aimed to analyze patients with SCI and serum sodium in 14/1 days post-trauma, treatment and prognosis. Method: This study is a prospective, longitudinal, descriptive realized in Emergency Hospital of Sergipe (HUSE), Department of Neurosurgery. Results: It was prevalent among males (80%). There is higher incidence in the age group below 40 years, with an average age of 38.4. The level of injury has relationship with the onset of hyponatremia. It was not necessary to impose any treatment. There were no deaths in the first two weeks. ABSTRACT

O manejo da hiponatremia é um desafio, pois não é possível estabelecer a conduta adequada apenas a partir do diagnóstico laboratorial. Para isso, é preciso ter conhecimento básico a respeito do equilíbrio da água corporal, e avaliar com cuidado o quadro clínico do paciente. Este artigo visa orientar o manejo da hiponatremia, principalmente em contexto hospitalar.


The management of hyponatremia is a challenge, since is not possible to establish a proper approach using only laboratorial parameters. In order to manage it correctly, basic knowledge regarding water balance and careful evaluation of the patient's clinical status are needed. This article aims to provide guidance for the management of hyponatremia, focusing mainly in inpatient treatment.

ABSTRACT

Alterations in plasma osmolality are related to changes in cell volume, which are the pathophysiological substrate of serious diseases. Under normal conditions sodium is the main determinant of plasma osmolality, and its homeostasis depends primarily on water balance. Hyponatremia is common in clinical practice, and is associated with morbidity and mortality by itself or in relation to its treatment. Despite this, sodium disorders are underdiagnosed, undertreated, and often handled improperly. Because multiple conditions with different treatments can be manifested through hyponatremia, the clear understanding of the pathophysiologic condition of each patient is essential for proper management. ABSTRACT

Ascites and dilutional hyponatremia are frequent conditions in cirrhotic patients and its occurrence has a worse prognosis. Thus, patients with ascites should generally be considered for referral for liver transplantation. The present article explores issues related with ascites classification and the treatment corresponding to each of the 3 degrees of ascites. Regarding refractory ascites, recommendations are included on the use of diuretics, large volume paracentesis with the administration of intravenous albumin and use of transjugular intrahepatic portosystemic shunt (TIPS). Finally, physiopathological aspects involved in the development of dilutional hyponatremia and vaptans treatment, are presented, which are aquaretics that selectively blockade vasopressin V2 receptors in the principal cells of the collecting ducts, being one of the most innovative pharmacological interventions for the management of hyponatremia in cirrhotic patients, in the recent years.


La ascitis e hiponatremia dilucional son complicaciones frecuentes en pacientes con cirrosis avanzada y su aparición implica un peor pronóstico. Por ello, los pacientes con ascitis deberían ser referidos para trasplante hepático. El presente artículo explora los aspectos relacionados con la clasificación de la ascitis y el tratamiento correspondiente a cada uno de los 3 grados de ascitis. En relación al manejo de la ascitis refractaria, se incluyen recomendaciones respecto a la eficacia del uso de diuréticos, paracentesis evacuadora radical con reposición de albúmina intravenosa y uso de cortocircuito porto-sistémico intrahepático (TIPS). Finalmente, se abordan los aspectos fisiopatológicos involucrados en el desarrollo de hiponatremia dilucional y el tratamiento con vaptanes, que son acuaréticos que bloquean selectivamente los receptores V2 de vasopresina en las células principales del túbulo colector, constituyendo una de las intervenciones farmacológicas más innovadoras en el manejo de la hiponatremia en cirrosis en los últimos años.

JUSTIFICATIVA E OBJETIVOS: Definir e atualizar protocolos de tratamento e condutas em pacientes com hiponatremia na emergência médica com enfoque no diag¬nóstico causal da doença e métodos de correção do sódio sérico, consultando estudos de revisão sobre hiponatremia e distúrbios hidroeletrolíticos dos últimos 10 anos nas bases de dados LILACS, MedLine, BIREME, N England J Med e Lancet, a fim de estabelecer um protocolo de tratamento de hiponatremia na emergência. A grande prevalência da hiponatremia na emergência e como complicação aguda de inúmeras doenças, justificada pela grande dificuldade encontrada na correção dos eletrólitos de forma eficaz e que melhorem o prognóstico do paciente quando tratado precocemente. CONTEÚDO: A hiponatremia é caracterizada como o valor de sódio sérico menor que 136 mEq/L, sendo o distúrbio eletrolítico mais freqüentemente encontrado na prática clínica, presente em 6% dos pacientes internados. O tratamento da hiponatremia leva em consideração a classificação dos seus tipos, quanto a sua osmolalidade e em alguns casos à relação com a volemia do paciente, sendo feita na maioria dos casos de forma não controlada e com valores aproximados, não levando em conta outras características dos pacientes. Decorrente da grande dificuldade de se estabelecer a forma mais correta de se corrigir a volemia do paciente e a reposição do sódio, a elaboração de um guia de procedimentos e condutas a serem adotadas se justifica meio a grande prevalência da condição em pacientes portadores de doenças agudas e crônicas e frente à dificuldade de se estabelecer o tratamento mais efetivo nessas condições. As terapêuticas disponíveis seja a restrição hídrica, infusão de soluções ou uso de diuréticos são frequentemente ineficazes e com resultados nem sempre previsíveis, com potenciais efeitos secundários como a mielinólise pontina central. CONCLUSÃO: A hiponatremia é uma condição clínica comum na prática médica, sendo uma emergência responsável por altas taxas de complicações em pacientes internados. Pode manifestar-se de diferentes formas e devem ser prontamente reconhecidas pelo médico a fim de se estabelecer a correção do distúrbio eletrolítico, assim como iniciar o tratamento da doença de base do paciente. O objetivo de o tratamento ser a correção dos eletrólitos deve-se tratar cada individuo separadamente, levando em consideração as comorbidades e condições clínicas apresentadas no momento do tratamento.(AU)


BACKGROUND AND OBJECTIVES: Define and up-date treatment protocols and behaviors in patients with hyponatremia in medical emergencies with focus on casual diagnosis of the disease and methods of correction of serum sodium rising review articles on hyponatremia and electrolyte disturbances in the last 10 years in databases such as LILACS, MedLine, N England J Med, and Lancet in order to establish a protocol for treatment of hyponatremia in the emergency. The high prevalence of hyponatremia in emergency and as a complication of many acute diseases, justified by the great difficulties encountered by medical in the correction of electrolyte effectively and to improve the prognosis when treated early. CONTENTS: Hyponatremia is characterized as the value of serum sodium less than 136 mEq/L, being the most common electrolyte disorder encountered in clinical practice, affecting 6% of hospitalized patients. The treatment of hyponatremia takes into account the classification of types of hyponatremia and its osmolality and in some cases the relationship with the patient blood volume, being made in most cases in an un¬ controlled way and with approximate values, not taking into account other features patients. Due to the great difficulty in establishing the best thing to correct the patient?s blood volume replacement and sodium, the preparation of a guide to procedures and measures to be adopted is justified through the wide prevalence of the condition in patients with acute and chronic and facing difficulty in establishing the most effective treatment in these conditions. The available treatment is fluid restriction, infusion solu¬tions or diuretics are often ineffective and results not always predictable, with potential side effects such as central pon¬tine myelinolysis. CONCLUSION: Hyponatremia is a common clinical problem in medical practice, and an emergency responsible for high rates of complications in hospitalized patients. Hy¬ponatremia can manifest itself in different ways and should be readily recognized by the physician in order to establish the correction of electrolyte disturbances, and initiate treatment of the underlying disease of the patient. Even the goal of treatment is correction of electrolytes should be to treat each individual separately, taking into account comorbidities and clinical conditions presented at the time of treatment.(AU)

ABSTRACT

Severe hyponatremia occurring as the presenting feature of hypopituitarism secondary to pituitary adenomas is rare. We report three patients with this condition: Two elderly males (74 and 78 year-old) presenting with impaired consciousness and low plasma sodium after an episode of diarrhea and a 56-year-old male presenting with impaired consciousness after an episode of vomiting. All had clinical features of hypopituitarism and pituitary adenomas were found on imaging studies. Two were subjected to a trans sphenoidal resection of the adenoma.

ABSTRACT

OBJETIVO: Examinar as necessidades hídricas (água livre de eletrólitos) a serem consideradas quando da administração de fluidos de manutenção na criança em estado grave. Analisamos algumas das dificuldades na estimativa desses requisitos, e discutimos as controvérsias a respeito das recomendações tradicionais. FONTES DOS DADOS: MEDLINE (1966-2007),Embase (1980-2007), e Cochrane Library, usando os termos: “fluidoterapia”, “solução hipotônica”, “isotônica”, e sinônimos ou termos relacionados. SíNTESE DOS DADOS: A solução de manutenção e regime de fluido ideais permanecem um ponto de controvérsia em pediatria. As recomendações tradicionais para fluidos de manutenção são cada vez mais criticadas por não se aplicarem consistentemente à doença aguda, onde o gasto de energia e a necessidade de eletrólitos se desviam significantemente das estimativas originais. Apresentamos uma estrutura fisiologicamente fundamentada para a prescrição de fluidos de manutenção, com o objetivo de manter a tonicidade em equilíbrio, e administrar o volume mínimo de fluidos de manutenção necessário para manter a hemodinâmica. Discutimos também as indicações para soluções isotônicas e hipotônicas. CONCLUSÕES: As prescrições de fluidos de manutenção devem ser individualizadas. Não existe uma solução endovenosa única e ideal para todas as crianças durante todas as fases da doença, mas há evidências suficientes para sugerir que a escolha empírica mais segura é uma solução isotônica. As soluções hipotônicas devem ser consideradas apenas se o objetivo é alcançar um equilíbrio positivo de água livre. As crianças em estado grave podem necessitar de uma redução de até 40-50 por cento dos volumes de manutenção atualmente recomendados. Todos os pacientes que receberem fluidos endovenosos devem ser monitorados bem de perto, com pesagem diária, equilíbrio dos fluidos, controle dos parâmetros bioquímicos e clínicos, a fim de melhor orientar esta terapia.


OBJECTIVE: To examine electrolyte-free water requirements that should be considered when administering maintenance fluids in a critically ill child. We examine some of the difficulties in estimating these requirements, and discuss the controversies with respect to the traditional recommendations. SOURCES: MEDLINE (1966-2007), Embase (1980-2007), and the Cochrane Library, using the terms: “fluid therapy”, “hypotonic”, “isotonic solution”, and synonyms or related terms. SUMMARY OF THE FINDINGS: The ideal maintenance solution and fluid regimen remains a topic of heated debate in pediatrics. The traditional recommendations for maintenance fluids are increasingly criticized as they do not consistently apply in acute illness, where energy expenditure and electrolyte requirements deviate significantly from the original estimates. A physiologically based framework for prescribing maintenance fluids is presented, with the objective of maintaining tonicity balance, and infusing the minimum volume of maintenance fluid required to maintain hemodynamics. Indications for isotonic and hypotonic solutions are discussed. CONCLUSIONS: Maintenance fluid prescriptions should be individualized. No single intravenous solution is ideal for every child during all phases of illness, but there is evidence to suggest that the safest empirical choice is an isotonic solution. Hypotonic solutions should only be considered if the goal is to achieve a positive free-water balance. Critically ill children may require a reduction by as much as 40-50 percent of the currently recommended maintenance volumes. All patients receiving intravenous fluids should be monitored closely with daily weights, fluid balances, biochemical and clinical parameters in order to best guide this therapy.

ABSTRACT

OBJECTIVE: To report a pediatric case of central pontine and extrapontine myelinolysis, a rare neurological disease often associated with rapid correction of hyponatremia.DESCRIPTION: A 15 year-old female adolescent developed locked-in syndrome during severe hyponatremia. Brain magnetic resonance imaging was consistent with the diagnosis of central pontine and extrapontine myelinolysis.COMMENTS: Serum sodium correction should proceed slowly and cautiously, based upon a careful calculation of sodium deficit, in order to minimize metabolic stress and avoid the occurrence of this dreadful complication, which has a tragic outcome in most cases. There is no scientifically proved effective treatment for myelinolysis, and severe cases usually have a dismal prognosis.

ABSTRACT

A hiponatremia é uma freqüente complicação do traumatismo craniano e surge, usualmente, nos primeiros dez dias depois do trauma e tem evolução limitada a poucos dias. Os autores apresentam um paciente que desenvolveu hiponatremia no nono dia após um acidente de motocicleta, a qual se prolongou por mais de vinte dias. Essa demorada hiponatremia pôde ocorrer porque o paciente foi tratado, inicialmente, com furosemida, como se tivesse a síndrome inapropriada da secreção do hormônio antidiurético, e com glicocorticóide, como se tivesse insuficiência glandular supra-renal. Contudo, o paciente teve síndrome cerebral perdedora de sal, que pode ocorrer após lesões do sistema nervoso central. Foi tratado com fludrocortisona (0,3 mg/dia) com reposição de sódio e de líquidos. Houve gradual desaparecimento da hiponatremia, e seu perfil físico e neural apresentou progressiva melhora. A fludrocortisona é um mineralocorticosteróide, e seu uso em pacientes com síndrome cerebral perdedora de sal é indicado pela ocorrência de secreção inapropriada do peptídeo natriurético cerebral, que inibe a secreção da aldosterona e, assim, provoca a perda renal de sódio.


Hyponatremia is a frequent complication following cranial traumatism and usually appears in the first ten days after the trauma, usually limited to a few days. The authors report a patient that developed hyponatremia on the 9th day after a motorcycle accident and it lasted twenty more days. The long period of hyponatremia could be explained by the treatment with furosemide, initially given with the assumption that he had inappropriate secretion of antidiuretic hormone syndrome, toghether with glucocorticoid, as if he had adrenal insufficiency. However, the patient had cerebral salt-wasting syndrome, which can happen after lesions of the central nervous system. Then, he was correctly treated with fludrocortisone (0.3 mg/day) and sodium plus fluid replacement. He gradually recovered from the hyponatremia and his physical and neural profile showed progressive improvement. Fludrocortisone is a mineralocorticosteroid and its use in a patient with cerebral salt-wasting syndrome is justified by its counter effect against the oversecretion of brain natriuretic peptide, which inhibits the aldosterone secretion, eventually leading to renal loss of sodium.
